Answer our three orthodontic trivia questions below and you’ll be entered to win a LEGO Creator 3-in-1 Cozy House. Build it as a cozy family home, then rebuild it into a canal house or a lakeside cabin — three houses in one set!
Entering is easy: fill out the form, take your best shot at the trivia, and hit submit. That’s it — you’re in the drawing!
One entry per patient. Winner will be drawn after the contest closes September 30 and contacted by their OA office.
How to Earn Reward Points
Arrive early or on time at scheduled appointments.
Wear elastics as instructed.
No loose or broken appliances or brackets.
Schedule your next appointment while in our office.
Invisalign patients are rewarded 1 point per aligner change and staying on track with treatment schedule.
Get an A or B grade for
maintaining good oral hygiene.
Do you volunteer? Bring us a signed letter from the service organization.
We reward good school grades! For kids, show us your A’s and B’s on a copy of your report card.
Visit your general or pediatric dentist for a regular check-up and cleaning. Bring your receipt to our office at your next appointment.

You will receive 10 points at your new patient consultation with us.
