Smile Photo Contest
Submit your smile photo below for a chance to win $100 CASH! One winner per OA office! Each patient who enters will receive one $5 gift card to Sonic, while supplies last.
- Your photo can be a selfie or portrait! Whatever shows us your happy, OA smile! Take the photo at home, at the park, on summer vacation, at the pool, or even at your next OA appointment – wherever your smile shines the brightest!
- Contest ends July 31, 2026. One winner from each office will be chosen at the beginning of August. Each patient may submit more than one photo, but you will only receive one $5 drink card.
- Take the photo in the morning or evening hours when the light outside is best for photos! If inside, make sure you are facing window light.
- Send original photos from your smartphone or photographer. Please no screenshots.
- Make sure the photo is not too dark or blurry.
- If you send us a photo from a professional photographer, please let us know the name of the photographer so we can give them credit on social media.
- When you enter the contest, we will make a note in your chart that you earned a drink gift card. When you come in for your next appointment, we will give you the card. If you’d like to drop by sooner, just call and let us know!
- When submitting a photo, you will be asked to check mark that you read a media release form giving us permission to use you or your child’s photo on social media. It is not guaranteed that you or your child’s photo will be posted on social media.

No loose or broken appliances or brackets.
Schedule your next appointment while in our office.
Invisalign patients are rewarded 1 point per aligner change and staying on track with treatment schedule.
Get an A or B grade for
maintaining good oral hygiene.
Do you volunteer? Bring us a signed letter from the service organization.
We reward good school grades! For kids, show us your A’s and B’s on a copy of your report card.
Visit your general or pediatric dentist for a regular check-up and cleaning. Bring your receipt to our office at your next appointment.
Check in on Facebook during each office visit, and let us know.
Wear your OA t-shirt to your appointment or tag us on social media wearing your shirt!
You will receive 10 points at your new patient consultation with us.
Observation patients are rewarded 10 points at each check up.

Refer a friend to our office! If your friend mentions your name at their new patient consultation, you will receive points at your next appointment.
